Enhanced Home and Community Care Services (Sham Shui Po)

History

Tung Wah Group of Hospitals Enhanced Home and Community Care Services (Kowloon City/Yau Tsim Mong/Shamshuipo) received subsidy from Social Welfare Department in October 2008 and commenced operation in December 2008. It provides alternatives apart from placement at residential homes for frail elders to continue living in the community with comfort and dignity while receiving a wide range of home care services.

The Social Welfare Department has regularised the Enhanced Home and Community Care Services from September 2020. The Centre, renamed as Enhanced Home and Community Care Services (Sham Shui Po), provides home care services for the elderly living in Sham Shui Po District.

Objective

Enhanced Home and Community Care Services adopts an integrated mode of services to meet the nursing and personal care needs of frail elders with the aims of enabling them to age at home, maintaining their maximum level of functioning and providing support to carers in order to strengthen family cohesion.

Service Recipients

• Living in Sham Shui Po District
• Aged 60 or above living in the community
• Elderly persons assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services, and have the genuine need for a well-coordinated package of home care and community support services
